Exemption Request Form

Per "General Rules, Section H (STARTING POSITIONS/PARTNERS), par. 4," club members are eligible for two (2) exemptions from the draw per tournament season (April thru March). If you wish to use an exemption, please fill out the form below and submit.  Also, if you wish "Confirmation," please check the box provided and you will receive an email confirming your request.

Note: Any requests must be submitted (in writing if not using this form) prior to the meeting of the month requested to be considered.
